Understanding the diagnoses page

Here, we list all diagnoses that have been added to your record. For current diagnoses, we display the most recently added first. For past diagnoses, we display the most recently ended first.

Click on a diagnosis to show who entered the information and when. We will also show coding information if we have it.

How to add a diagnosis

Click ‘Add diagnosis' and enter information about the diagnosis.

Entering an end date

End dates should be left blank if you still have the problem. Do not estimate or add an end date that hasn’t happened yet.

How to change a diagnosis

Click 'Edit' or 'Delete' beside the diagnosis. You can only edit or delete diagnoses that you have entered yourself. If you have not entered the diagnosis but need it to be changed, please see our Reporting Incorrect Data page.

How to change the privacy of a diagnosis

To edit a privacy click on the pencil icon beside the privacy label of the diagnosis and select the new privacy label. Please note: Changing the privacy may result in some professionals and carers no longer being able to view the diagnosis.

How to discuss a diagnosis

Select one or more diagnosis and click 'Discuss selections'.
