Introduction

In 2020, St Patrick’s Mental Health Services (SPMHS) became the first mental health provider in Ireland to adopt Patients Know Best (PKB) as its Personal Health Record (PHR) platform. Within SPMHS, the platform is known as Your Portal.

A Digital Solution for Mental Health

SPMHS places service users at the centre of their care. PKB supports this mission by providing a secure, user-friendly digital platform that enables individuals to access their health records, communicate with care teams, and actively participate in their treatment.

Integrated with SPMHS’s electronic health record system (eSwift, built on the Rio platform), PKB enables seamless information flow across inpatient services, day programmes, and home-based care, supporting a unified, digital-first approach to mental healthcare.

Co-Designed with Service Users

Your Portal is continually shaped by feedback from service users, particularly through the Remote Care User Advisory Forum, which meets monthly to guide digital development and ensure the platform meets the real-world needs of users.

Integrated with eSwift and the SPMHS Business Intelligence tool, the system streamlines workflows for pre-registration, appointment management, and document sharing.

Appointment

Appointment details, including Microsoft Teams video links when applicable, are shared instantly, ensuring timely communication.

Questionnaires

Clinical assessments sent via eSwift are completed by service users and automatically returned, enabling data-driven decisions across care pathways such as Homecare, Day Programmes, and therapeutic assessments.

Care Plans and Clinical Documents

Multidisciplinary care plans, key worker summaries, discharge summaries, and post-appointment reviews are shared securely via the Events & Messages or Plans sections.

Single Sign-On (SSO)

Clinicians and administrative staff can upload documents directly to a user’s record from within eSwift.

Resource Library

A curated digital library helps service users navigate mental health services and better understand their rights and care options within the Irish healthcare system.
