This workflow helps patients complete their pre-op assessment forms in Patients Know Best (PKB), and makes their answers easy for clinicians to view in iPro during their appointment.

1. Organisation sends questionnaire to patient

The process starts when the hospital sends a questionnaire using PKB’s APIs, asking the patient to fill out their Pre-Op Assessment questionnaire. PKB shows the questionnaire to the patient in the Events & messages section in the web user interface and in the NHS App.

2. Patient fills out the questionnaire

The patient completes the form at a time that suits them. They can do this by logging into PKB directly; clicking the link in the PKB email notification; or through an SMS if the organisation sent one.

All patients can answer the questionnaire, whether or not they have completed registration for PKB.

3. Clinicians can view the responses in iPro

Once the patient finishes the form, iPro pulls the answers automatically from PKB. When the patient arrives for their appointment, the clinician can easily see the patient’s answers in iPro.

In the screenshot below, the answers are displayed within iPro.

4. The responses stay available in PKB

Patients can always see in PKB their answers in the PKB user interface — PKB stores the completed questionnaire in their record for future reference.
