  • Habit tracking

  • Engaging content

• Sustainable Health Improvements – A structured approach to enhancing both mental and physical well-being.

This patient-centered tool empowers users to make meaningful, long-term changes to their health.

Holly Health has partnered with over 200 GP practices in England, Scotland and Wales. The cost of Holly Health is typically funded by the PCN or ICS, enabling free access to the end user for at least 12 months. It is often offered to all adults in a population group and is particularly useful for people living with, or at risk of, long term conditions like hypertension, anxiety, depression, type 2 diabetes, etc.

How to connect Holly Health with PKB

Go to the ‘Today’ page in the Holly Health app.

Then tap Account settings (top right icon).

Under Account settings, tap ‘Connect with Patients Know Best’ Orange button.

The Holly Health chat flow will open and take the user through the steps to connect with PKB.

If already registered with PKB

Enter your email address and password for PKB and click ‘Approve’ to connect Holly Health and PKB.

Complete the registration process through NHS Login, and then return to the Holly Health app.

Click ‘I’ve registered’ and then ‘Connect to Patients Know Best’.

Holly Health results sent to PKB

Blood pressure readings entered in the Holly Health app, along with a PDF summary of your health data, are automatically synced to PKB’s Measurements and Plan sections. This information is also accessible within the NHS app, ensuring seamless integration and easy access to your health insights.
