PKB Care Plan Request Process

PKB offers a library of care plan templates that teams already use with their patients. If you want to create a care plan template for your team, PKB supports the creation of three templates, with two updates per plan.

To request a care plan template, download or copy the PKB care plan template to add your content, then send your request to your Success PM. They will submit it for internal review and contact you within five working days.
