Decision Support Tools

NHSE has created these decision support tools and has been developed in accordance with The National Institute for Health and Care Excellence standards framework for shared decision-making support tools, including patient decision aids.

PKB has converted these decision support tools into digital, editable and shareable care plans for people to access when making decisions about their care. The benefit for patients having them in their PKB is that they can access them at any time and share them with whom they need to. They can also edit the plan as and when needs or decisions change.

How To Use Them

Organisations

These decision support tool templates can be used by trusts focusing on elective recovery pathways. Add them to the Patient Record Team central library, Waiting List Validation admin team, and eMeet and Greet team library.

Teams

Speciality teams can add the templates to the team's library and promote them to patients or discuss them in face-to-face or virtual appointments. It empowers people to make decisions about the right care for them. An ideal team to use these plans is any team doing any waiting well pathways.

Patients

Adds the care plan template to the plans section in the PKB record, allowing them to edit and share the plan with anyone involved in their care. Giving patients access to digital decision support tools that can be the focus of their appointments. Treatment plans that involve the patient, even if that treatment plan is for none.
