When creating a care plan in PKB for your patients, it's useful to use the provided template, ensuring you utilise all its functionalities and understand how patients can interact with it once it's’s created.

PKB has put together the care plan template below, which teams can download or copy and add their content.

After adding the content to the care plan template, share it with your Success project manager to make sure every section has been utilised, and any blood tests added have the right values.

The care plan template can be submitted by sending the request to your Success PM and they will submit it internally.
