Why
Using DIALOG questionnaires helps create a more collaborative and patient-centred approach to a patient's health and social care, leading to better outcomes and patient satisfaction.
PKB Digital DIALOG questionnaires allow individuals to complete them at home, with the support of their loved ones or caregivers. This approach places the individual at the centre of their care, enabling them to review and reflect on the questionnaires at any time. Additionally, these questionnaires can be shared with anyone involved in their care or who is relevant to their situation.
What
The DIALOG assessment consists of 11 questions that enable individuals to assess their satisfaction in eight areas of their life and three treatment aspects using a 7-point scale. This evaluation provides scores for quality of life and treatment satisfaction. While the scale is a key part of the DIALOG intervention, it can also be used independently.
How
PKB has digitised the DIALOG and DIALOG+ questionnaires for teams to use with patients. These digital questionnaires can be completed directly from a patient's PKB home screen or sent by a healthcare professional or coordinator before or during an appointment.

Measurable: Reduce the percentage of "missing" or "incomplete" Dialog Plus to less than 5% by ensuring 100% of completed questionnaires are automatically pulled into the Trust's EPR (e.g., Rio).
Achievable: Use PKB's integration capabilities to push completed assessment data directly into clinical systems, eliminating manual data entry.
Relevant: Standardises documentation across the organisation and reduces the administrative burden on clinicians compared to manual paper tracking.
Time-bound: Complete the digital integration and baseline audit within three months of implementation.
3. Support crisis prevention and DNA reduction
Specific: Use the Dialog Plus snapshots (at the three treatment stages) to proactively identify high-risk patients and reduce appointment "Did Not Attend" (DNA) rates.
Measurable: Reduce DNA rates by 5% for the cohort of patients actively using digital Dialog Plus on PKB compared to the pre-PKB baseline.
Achievable: Enable patients to view and manage appointments within PKB where they complete their assessments, keeping them more engaged in their care journey.
Relevant: Directly impacts patient safety by ensuring snapshots of feelings are documented and clinicians are alerted to potential crises earlier.

4. Increase digital adoption and patient registration to PKB
