Mental Health Organisation Rollout of PKB - Sussex Partnership NHS Foundation Trust
In 2021, Sussex Partnership NHS Foundation Trust initiated the rollout of PKB to enable their service users interact with their health data and engage with their care teams. Importantly, it allowed them to share parts of their records with others involved in their care, including their GP, other NHS Trusts, care workers, friends, and family carers.

Workflow
Midlands Partnership NHS Foundation Trust
In 2023, Midlands Partnership NHS Foundation Trust (MPFT) embarked on a transformative journey with PKB in patient empowerment, particularly in the context of Adult Mental Health Services (AMHS) and Making Every Contact Count (MECC).
The aim was to give people access to their health records, enabling self-management of care to improve their experience and reduce service demand.
Features
Registration and PROM Completion:
Service users receive an email prompting them to register and complete their PROMs through the PKB platform.
Completion of PROMs:
Service users fill out the relevant PROMs questionnaires within the secure PKB portal.
Integration with RiO:
Once completed, the PROMs responses are automatically fed back into RiO, which clinicians use to allow immediate access to the latest data.
Clinician Review and Care Plan Development:
Clinicians use the PROM data to assess the service user’s current health status and tailor their care accordingly, which can be achieved by incorporating the insights gained into developing individualised care plans.
Sharing with Service Users:
Which can be reviewed and tracked so that they remain engaged and involved in their treatment and support journey.

Outcomes

Auto Trigger FHIR API of QA’s (with that appointment) (repeated with each appointment)
The FHIR API triggers questionnaires (QAs) with each appointment automatically.
When a patient has an appointment, the system automatically generates relevant questionnaires for them to complete, and the questionnaires are repeated with each patient appointment
Ensures the information collected is up to date and relevant for ongoing care planning.
This approach enhances data collection efficiency by ensuring that PROMs are linked to specific appointments, reducing the need for manual intervention
