Benefits of PKB PROMs

Benefits of PKB PROMs

Why

Patient-reported outcome Measures (PROMs) are critical tools in healthcare that offer valuable insights into a patient's perception of their health, well-being, and the effectiveness of their treatment. PROMs are versatile tools used by any clinical team and across all specialities, enabling more patient-centred care and informed decision-making.

What

PKB offers a secure, efficient, and patient-centred platform for PROM's data collection. Its advanced digital questionnaire functionality provides a comprehensive solution for capturing, managing, and analysing patient-reported data.

Key Features

  • Scoring and calculations: Automates scoring for standardised measures to enhance efficiency and accuracy.

  • Branching logic: Dynamically tailors questionnaires so patients only complete sections relevant to their situation.

  • Coded field mapping: Supports mapping questionnaire responses to clinical codes for seamless integration.

  • Save drafts: Patients can save progress and return later to complete their questionnaires.

  • Flexible delivery: Teams can send questionnaires to patients regardless of whether they are registered to PKB or already part of a team.

  • Administrative efficiency: Reduces the workload and errors associated with traditional paper-based methods, improving data accuracy.

    • Reduced workload:

    • Reduced errors:

  • Customisable: Tailor questions to align with specific clinical pathways or service requirements.

  • Integrated experience: Embedded within PKB, enabling patients to complete their questionnaires alongside other health management tasks. This could include viewing their health results and sharing data or messaging their care team.

How

Use the Advanced Questionnaires feature in PKB to create and customise PROMs surveys, collect responses, and analyse results to enhance patient care.

  • Invite patients: Encourage patients to complete their PROMs securely in PKB.

  • Increase registrations: Promote PKB registrations to increase the number of patients who can securely complete their PROMs surveys

  • Create tailored questionnaires: Use PKB’s library of PROMs examples to design templates tailored to your team’s needs.

  • Adapt your workflow: Integrate PKB’s digital questionnaires into your existing PROMs processes.

• Flexible sharing options: Share/send questionnaires digitally via email, within the PKB platform, or through the NHS App. PKB offer multiple choices:

    • Email: Send questionnaires directly to patients via email with secure access links

    • PKB Platform: For patients already registered on PKB, send questionnaires through their PKB account, providing an integrated experience

    • NHS App: For patients using the NHS App, questionnaires are shared directly, further enhancing accessibility

  • Integrate with existing systems: Use PKB’s API capabilities to incorporate PROMs data into clinical pathways and systems.

  • Patient convenience: Patients can complete questionnaires at their convenience using any device.

  • Analyse data: Access real-time results and generate reports to inform care decision-making.

PKB PROMs

PKB’s digital PROM questionnaires are versatile and adaptable. Below are some examples of where they can be implemented:

  • Chronic disease management: Monitor the ongoing impact of long-term conditions like diabetes or COPD.

  • Surgical pathways: Assess recovery progress and patient satisfaction post-surgery.

  • Mental health: Capture insights into patient well-being and therapy outcomes.

  • Clinical trials: Collect consistent and reliable data directly from patients.

  • Hospitals: Use PROMs to benchmark services and drive continuous improvement.

  • PIFU (Patient-Initiated Follow-Up): Streamline follow-up processes by gathering patient-reported data.

  • Oncology: Evaluate treatment effectiveness and patient quality of life during and after cancer care.

Benefits of using PKB for PROMs

  • Secure and efficient data collection, completed within the PKB portal.

  • Real-time insights allow for timely intervention

  • Patient-centred care, incorporation of the patient's voice into clinical care planning.

  • Standardised PROMs ensure consistency across teams and or organisations.

  • PKB care plan templates for tailored interventions.

Use Cases

Mental Health Organisation Rollout of PKB - Sussex Partnership NHS Foundation Trust

In 2021, Sussex Partnership NHS Foundation Trust initiated the rollout of PKB to enable their service users interact with their health data and engage with their care teams. Importantly, it allowed them to share parts of their records with others involved in their care, including their GP, other NHS Trusts, care workers, friends, and family carers.

Features

  • Appointment letters, appointments, and care plans are sent to Patients with a mental health privacy label  (for more information about our privacy labels click here).

  • Recall questionnaires through an API

Outcomes

  • All active adult users have been successfully added to PKB

  • 36% of patients registered

  • Delivery of questionnaires, even if the patient is not registered

  • Several widely used and relevant PROMs are already integrated into the platform, including:

    • GAD7 (Generalised Anxiety Disorder Scale)

    • PHQ9 (Patient Health Questionnaire for depression)

    • DIALOG and DIALOG+ (tools for assessing the quality of life in people with severe mental illness (SMI))

  • High engagement when linking PROMS platform to wider Health Record access

Workflow

Midlands Partnership NHS Foundation Trust

In 2023, Midlands Partnership NHS Foundation Trust (MPFT) embarked on a transformative journey with PKB in patient empowerment, particularly in the context of Adult Mental Health Services (AMHS) and Making Every Contact Count (MECC).

The aim was to give people access to their health records, enabling self-management of care to improve their experience and reduce service demand.

Features

  1. Registration and PROM Completion:

  • Service users receive an email prompting them to register and complete their PROMs through the PKB platform.

  1. Completion of PROMs:

  • Service users fill out the relevant PROMs questionnaires within the secure PKB portal.

  1. Integration with RiO:

  • Once completed, the PROMs responses are automatically fed back into RiO, which clinicians use to allow immediate access to the latest data.

  1. Clinician Review and Care Plan Development:

  • Clinicians use the PROM data to assess the service user’s current health status and tailor their care accordingly, which can be achieved by incorporating the insights gained into developing individualised care plans.

  1. Sharing with Service Users:

  • Which can be reviewed and tracked so that they remain engaged and involved in their treatment and support journey.

You can read more about the project here.

Outcomes

  • Currently, 21% of patients are registered with PKB

  • Auto Trigger FHIR API of QA’s (with that appointment) (repeated with each appointment)

  • The FHIR API triggers questionnaires (QAs) with each appointment automatically.

    • When a patient has an appointment, the system automatically generates relevant questionnaires for them to complete, and the questionnaires are repeated with each patient appointment

    • Ensures the information collected is up to date and relevant for ongoing care planning.

    • This approach enhances data collection efficiency by ensuring that PROMs are linked to specific appointments, reducing the need for manual intervention

PROMS Questionnaire Templates

PKB developed a variety of PROMs advanced questionnaires for our customers to use with their patients. The questionnaires help improve patient care and guide clinicians in tailoring treatment plans.
