It supports shared decision-making, allowing patients to record their preferences for future care and treatment.

In England, PKB integrates with GP systems and automatically pulls in medical records for registered patients. It provides a secure, digital platform where patients, carers, and clinicians can access and share crucial care information.

Current Baselines

Currently, ReSPECT plans are typically paper-based, which limits access during emergencies and may result in decisions that do not accurately reflect the patient’s wishes. PKB transforms this by making the ReSPECT plan securely and digitally available whenever and wherever it is needed.

Workflow

  1. Patient: Claims their PKB record via NHS Login

  2. GP practice: Add carer to patient's record

  3. Carer: Claims their patient's PKB record. Ability to view the relatives/friends record as a carer.

  4. Patient: Attends appointment with GP

  5. GP: Adds the ReSPECT care plan to the patient's record

  6. GP: Adds and completes ReSPECT care plan with patient wishes

  7. Carer/Patient: Ability to view and add information to the ReSPECT care plan

  8. PKB: Displaces audit trail of any changes to the ReSpect care plan

  9. Patient/carer: Uses care plan to view wishes and share in emergency situations

  10. Emergency teams: Have the ability to view patients' ReSPECT care plan and GP and hospital data in the patients record.
