The Qrisk care plan has been adapted into a PKB care plan, providing patients with a copy of their care plan while allowing pharmacies and clinical staff to view relevant health data within their PKB record once the patient has given the team access to their PKB record. This initiative helps patients maintain a permanent and accessible record of their Qrisk assessment, enhancing collaboration among healthcare professionals and family members.

Why 

Community pharmacies use HealthTab's point-of-care testing devices to conduct screenings and manage various health conditions efficiently. HealthTab is directly integrated with PKB, allowing patients' test results and measurements, which have been recorded in HealthTab, to be shared directly into the patient's PKB record. These results can then be automatically pulled into populate the Qrisk Care Plan in the patient's record. 

Incorporating the Qrisk Care Plan into PKB makes this data accessible within a secure platform, improving care coordination. 

What

The Qrisk care plan in PKB allows patients to share their results with family members and anyone involved in their care.  To keep a permanent record of tests that are always accessible for future treatment by other healthcare professionals in a single data repository readily available to anyone with access, thereby facilitating true shared care.

How

Qrisk test results and measurements taken via Health Tab can be shared to a patient's PKB record via our partnership integration. Patient tests and measurements are taken via Health Tab, the patient can link their Health Tab account to their PKB record during the Health Tab sign-up process using NHS Login or their PKB login. 

These results will automatically be sent to PKB once processed. Clinical staff can then add the QRISK_3 risk care plan to a patient record when they are in clinic. The care plan contains the Qrisk test results, measurements and useful information which the patient can share with their family, friends and clinical teams. The Q in the care plan is used to determine an individual's risk of experiencing a heart attack or stroke within the next 10 years. The QRISK_3 score is calculated based on:

• UK National Health Service doctors and academics developed a validated algorithm.

• Data collected from thousands of general practitioners across the UK contributed to the QResearch database for medical research.

The care plan is added and completed during the appointment, ensuring informed medical decisions are shared with the patient. The patients can also view supportive information leaflets in the PKB Library for guidance on interpreting their results and managing their conditions.
