PKB has developed several diabetes care plans that our customers can use at various stages of their patient’s treatment. Each plan is designed to support and inform patients, enabling them to better manage their diabetes. If you would like to use or adapt any of the plans on this page, please consult your Success Project Manager.

Diabetes Annual Check Care Plan

Key Objectives

Provide patients with information about their annual diabetes appointment and give patients a digital care plan to track and monitor symptoms related to their diabetes, support patient empowerment.

Outcome Measures

Equip every patient with the information and advice to support the self-management of their diabetes. Educating them and giving them an escalation plan to follow when needed.

My Type 1 Diabetes Care Plan

Key Objectives

Provide patients with information about type 1 diabetes when newly diagnosed or waiting for a diagnosis. To give patients a digital care plan to track and monitor symptoms related to their diabetes and support patient empowerment.

<p>This care plan is designed to guide you if you've been diagnosed with Type 1 diabetes. It aims to help you manage your health, reduce complications, and live well.</p>
<p>Type 1 diabetes is a condition where your body cannot make a hormone called insulin. This causes the level of glucose (sugar) in your blood to get too high.</p>
<p>This plan is here to guide you, but every individual is different. Work with your healthcare team to tailor it to your needs.</p>

<p>Type 1 diabetes means your body can't make insulin, a hormone that helps turn glucose (sugar) into energy. Without insulin, blood sugar levels get too high.</p>

<p>Type 1 diabetes often starts in children or young adults but can happen at any age. It's an autoimmune condition with no current cure or way to prevent it.</p>
<p>(Type 1 diabetes is different from <a href="Type 2 diabetes " target="_blank">type 2 diabetes</a>, which is more common in older people and people who are overweight).</p>
<h3>What you need to know about Type 1 diabetes</h3>

<li>It's a lifelong condition where your immune system attacks the insulin-producing cells in your pancreas.</li>
<li>Your body can't produce insulin, so you must manage blood sugar with insulin therapy.</li>
<li>Early diagnosis and effective management can prevent complications.</li>

<h2>Recognising symptoms of Type 1 Diabetes</h2>
<h3>What to watch out for</h3>
<ul>
<li>Feeling very thirsty.</li>
<li>Needing to pee more often, especially at night.</li>
<li>Feeling very tired or lacking energy.</li>
<li>Unexplained weight loss or loss of muscle bulk.</li>
<li>Blurred vision.</li>
<li>Itching around your genitals or frequent episodes of thrush.</li>
<li>breath that smells sweet or fruity (like nail polish remover or pear drop sweets)</li>

<h2>What to do</h2>
<ul>
<li>If you notice symptoms, contact your GP as soon as possible.</li>
<li>In emergencies (e.g., confusion or unconsciousness from low blood sugar), call for immediate medical help.</li>
<li>Untreated symptoms can lead to a serious condition called diabetic ketoacidosis.</li>
</ul>

<h2>Call 999 or go to A&E immediately if you:</h2>

<li>Have been peeing more, feeling very thirsty and tired, and:

<li>Have stomach pain, diarrhoea, or are feeling/being sick</li>
<li>Feel sleepy or confused</li>

<p>Managing Type 1 diabetes takes daily care, but you can live well with the right tools and support. Below are some of the things you can do to manage your diabetes.</p>
