PKB Medication Optimisation

PKB Medication Optimisation

Patients Know Best (PKB) supports medicines optimisation by helping patients and healthcare teams collaborate effectively on medication management. Through features such as the Rule Engine and Banner Engine, PKB automatically identifies patients who may benefit from specific interventions and delivers tailored, actionable guidance directly within their digital health record.

Why Choose PKB

By embedding optimisation within digital systems like PKB, healthcare organisations can improve patient safety, clinical outcomes, and operational efficiency.

PKB offers a connected, patient-centred platform for medicines optimisation that enables, improved medication adherence through better communication and patient engagement and making sure patients are on the correct medication for the appropriate length of time.

What

PKB Meds Optimisation utilises PKB’s key functionalities, designed to empower healthcare professionals and patients to collaborate on medication management. It goes beyond simple e-prescribing by integrating real-time patient data, clinical decision support, and patient engagement features to ensure medications are not only prescribed but also taken effectively and safely.

By combining these two features, PKB empowers healthcare organisations to move beyond passive record-keeping. They can create a dynamic, responsive digital experience that guides and supports patients in managing their health, ultimately leading to better outcomes and a more efficient healthcare system.

How

PKB Meds Optimisation works by integrations with existing hospital and primary care systems (EPRs/EHRs) to create a single, up-to-date medication list that is shared with the patient in their PKB record, under Medicines. Patients also have the ability to add their own medicines into their record.

The Rules Engine and Banner

The Banner Engine is a specific component of PKB  that is directly controlled by the Rule Engine. Its purpose is to present targeted, context-sensitive messages (banners) to patients in their Personal Health Record.

How it Works

  1. A Rule Engine action is configured to display a banner when certain criteria are met.

  2. A customisable message appears on the patient’s record, on the medicines screen. It can include colours, links, or calls to action.

  3. The banner delivers timely, relevant information or prompts the patient to take a specific action.

Examples Banner and Rule Engine

  • Rule: If a patient's blood pressure readings are consistently high...

Action: ...THEN display a banner that says: "Your recent blood pressure readings are elevated. Please contact your GP practice to arrange a medication review/or request a certain type of medication"

  • Rule: IF a patient has a new prescription for a medicine that requires monitoring...

Action: ...THEN display a banner that says: "You've been prescribed a new medication. Click here to learn more about how to take it and what to expect."

  • Rule: If a patient has a condition added as a diagnosis, “Gout”

Action: THEN, displace a banner that says: You have a diagnosis of Gout. Are you on this medication?
