Team Self Registration

Why use Team Self-Registration

Your team can enable patients to self-register and add themselves to your PKB team. Once registered, patients gain access to the resources you choose to share, such as questionnaires, care plans, and helpful materials in your team’s PKB library, including documents, videos, and signposting links.

Team self-registration is especially useful for:

  • Self-referral services (e.g. maternity, physiotherapy)

  • "Waiting well" and PIFU (Patient Initiated Follow-Up) pathways, where patients self-monitor their condition

  • Clinical teams with limited administrative capacity

  • Charities providing open access to disease-related information

  • Research teams enrolling patients into clinical trials

What

You can promote self-registration by linking directly to your PKB team page in a variety of formats:

  • On your department’s webpage

  • In patient letters, leaflets, or posters

  • Via QR codes or direct links displayed in clinic spaces

On the department or team page, clearly explain why patients should register and what they can expect. This could include:

  • Service details
    Example: “You can self-refer for maternity care after 6 weeks of pregnancy.”

  • What you offer
    Example: “Access free, reliable information about your condition, provided by our team.”

  • What you need the patient to do
    Example: “Complete a short questionnaire to help us plan your physiotherapy.”

  • What the patient is consenting to
    Example: “By registering, you agree to take part in our research study.”

 How Team Self-Registration Works

The PKB platform automatically generates a unique web page for each team, using the following format:

https://SERVER.patientsknowbest.com/go/TEAMCODE

For example, on the UK server, the Torbay physiotherapy team has the screen shown in the screenshot below:

On the EU server, the CarePoint MedMij team has the screen shown in the screenshot below

For New Patients

On UK-based team pages, the “Register” button uses NHS login. This means new patients will be guided through verifying their identity via NHS login as part of the registration process.

For Existing PKB Users

Patients who already have a PKB record can link their record to your team in two ways:

  • Click the “Share” button within PKB and enter your team’s details.

  • Using their NHS login on the team page to connect their existing record directly.

To enable team self-registration for your service, contact your PKB Success Project Manager. They will provide you with your team’s unique registration link and support you in setting up any necessary resources (e.g. QR codes, web content, promotional materials).
